Is there a certain rank that you can achieve in each mission, depending on how well you go, or do you just achieve a rank per mission?
Does the game get harder, depending on how well you go?

You get a rank at the end based on your performance, but I don't know about the game getting harder....

There is a ranking system that takes, mission time, health and stealth into account.
The Top rank you can get is David Jones which has to be done on the hard setting.
Some of the times set were done by myself by the way and are possible
